Male Hypogonadism (American College of Physicians PIER)
Older men and men with chronic disease constitute the two largest subsets of androgen-deficient men, but the disorder often remains undetected because its symptoms in adult men are frequently nonspecific.

Hypogonadism may be a manisfestation of a serious underlying disease, such as a pituitary tumor or HIV infection. If left untreated, androgen deficiency may contribute to osteoporosis and increased risk for fracture; loss of muscle mass and function; impaired sexual function, mood, and energy level; increased fat mass, particularly in the visceral fat compartment, and insulin resistance.

This updated activity covers screening for androgen deficiency in specific populations; diagnosis through history and physical exam, as well as laboratory testing; consultation for diagnosis in certain cases; non-drug therapy in the form of counseling for androgen-deficient men who present with sexual dysfunction; drug therapy through testosterone replacement; patient education; consultation for management in the case of older men with very low baseline testosterone concentrations and patients who experience progressive or marked increases in PSA levels; and follow-up to monitor the effectiveness and safety of androgen replacement therapy. » Go to this activity
