Meningococcal Infection (American College of Physicians PIER)
Routinely immunize adolescents 11 to 18 years old with MCV4 at the earliest opportunity. Reserve the quadrivalent MPSV4 for children 2 to 10 years. Because neither are 100% effective in preventing disease from strains included in the vaccine and do not provide any protection against serogroup B strains, history of meningococcal immunization does not exclude the diagnosis of meningococcal infection.
Because the signs and symptoms of meningococcal disease are often nonspecific and cannot be clinically differentiated from other illnesses, the only way to definitively diagnose meningococcal infection is with specific laboratory testing. Consult an infectious diseases specialist and other specialists to assist in the diagnosis. All patients with suspected meningococcal infection should be hospitalized. Administer antibiotic chemoprophylaxis to close contacts of a patient with invasive meningococcal infection.
